MN Care Services is a certified Home and Community-Based Services (HCBS) 245D provider, offering personalized care to individuals with disabilities or those aged 65 and older. Our mission is to enhance the dignity, health, and independence of those we serve through a range of basic and intensive support services. We provide essential care, supervision, and assistance to ensure the health and safety of your loved ones. Our services include, but are not limited to, Comprehensive Rehabilitative Services (CRS), Home Support Services (HSS), Individualized Home Supports (IHS), Independent Community Living Skills (ICLS), Homemaker Services, Personal Support, and Respite Care, all in compliance with Minnesota Statutes Chapters 245A and 245D.

Currently serving as Chief Operating Officer, Jake's leadership spans a diverse range of roles, including Special Education Teacher, Designated Manager, and Executive Director. His extensive experience in program management, strategic planning, and operational oversight has been instrumental in driving organizational growth and enhancing service delivery.

With 16 years of experience in Disability Services, Natasha has built a comprehensive career that reflects her dedication and expertise. She began her journey as a Direct Support Professional (DSP), where she gained hands-on experience providing essential care and support. From there, Natasha advanced to the role of Behavioral Technician, further developing her skills in managing and supporting individuals with diverse needs. Ultimately, she achieved her goal of transitioning into an Administrative position, where she now applies her extensive knowledge to oversee operations and ensure the delivery of high-quality services. Natasha is deeply committed to helping those in need and continues to be a valuable asset to the field of Disability Services.

Anyone can enjoy our services as long as they have a physician’s order for the type of services they want. They must also have the consent of family members. For Medicaid and insurance paid services, a County Social Worker and/or Public Health Nurse’s assessment of need will be required as well. For private-pay clients, our RNs are able to do a thorough assessment of need following the physician’s order.
If you are an MA/Medicaid subscriber, then you will enjoy the support of a social or financial worker in your county. We do not offer financial work to qualify you but we can point you to the right office in your county to determine your eligibility. For private paying clients, yes, we would have sessions with you to agree your ability to pay and work out a mutually beneficial mode of payment.
