About us

MN Care Services is a licensed Home and Community-Based Services (HCBS) 245D provider, offering customized care to individuals both youth and adults with complex needs.  Our mission and vision is to provide limitless and effective services to members in the community to maintain their well-being.  We are striving to become a leading 245D provider and create a supportive environment for our individuals served. 

Our 245D services are designed to help you gain or reclaim independence, both at home and within your community. With the help of our highly skilled Direct Support Providers (DSPs), you can work towards your personal goals and aspirations. We take the time to understand what matters most to you and develop a personalized plan to guide you on your journey. Our team fosters engagement with your community and ensures you stay involved in meaningful activities. By trusting our DSPs with your care, you bring a sense of peace and confidence back into your life.


Services:

Community Residential Setting (CRS):

A licensed residential setting that serves adults and where the license holder does not reside. This setting typically uses a shift-staff model of support (i.e., paid staff work shifts on a 24-hour basis). In this type of setting, at least one person receives community residential services funded by an HCBS waiver program.

Currently operating Community Residential Services in Ramsey, Hennepin, Steele, and Winona Counties.

Child Foster Residence Setting (CFRS):

We offer continuous residential care and supportive services in corporate foster care settings for children under 18 who cannot remain in their biological or adoptive homes, where the license holder does not reside. These settings typically use a shift-staff model of support, with paid staff working shifts on a 24-hour basis. At least one person in these settings receives community residential services funded by an HCBS waiver program. 

Currently operating Child Foster Residential Services in Dakota, Goodhue, Steele, and Winona Counties.

24-hour Emergency Assistance (24hr EA):

On-call counseling, on-call problem solving and/or immediate response for assistance at a person's home due to a health or personal emergency that is not related to an injury or an immediate danger. This service does not replace the use of 911 when necessary.

Individualized Home Support (IHS):

Services for people who live in their own home or family home that provide support and/or training in the community living service categories listed in the covered services section. There are three types of IHS:

  • Without training.

  • With training.

  • With family training.

All types of IHS can be provided in the person’s own home, family’s home or in community spaces used by the public, and either in person or remotely.

Crisis Respite:

Short-term behavioral or medical intervention services to protect the person or others living with them and provide relief and support to the caregiver.

Brett Larsen

CEO

20 years of experience in the human services field. He has worked for profit & non-profit companies, been a director/Executive Director for 10+ years, and has specialized in creating opportunities for complex individuals. Brett has strong lead agency connections and lots of 245D knowledge/first hand experience. I look forward to partnering/working with you and finding solutions that work with any individual!

Jake Bauer

COO

With over 16 years of experience in the human services field, Jake Bauer has built a career dedicated to improving lives and systems in both educational and community settings. Currently serving as Chief Operating Officer, Jake's leadership spans a diverse range of roles, including Special Education Teacher, Designated Manager, and Executive Director. His extensive experience in program management, strategic planning, and operational oversight has been instrumental in driving organizational growth and enhancing service delivery.

Jake is deeply committed to fostering collaboration and innovation, with a focus on creating sustainable, positive outcomes for individuals and communities. Outside of his professional endeavors, he enjoys spending quality time with his friends and family, networking with industry professionals, and remaining actively involved in his community.


Christine Claiborne

Founder/President, MSW, LGSW 

With a profound passion for analyzing human interactions and studying individuals, groups, communities, cultures, and societies, I recognized the importance of creating a brand that goes beyond mere business ownership. MN Care Services was established to provide a unique service that not only maintains a person's independence but also builds upon their existing skills.

With over a decade of experience in human services, I have cultivated a network of professionals and experts dedicated to delivering person-centered services within the 245D community. The social and human services field is ever-evolving, offering continuous learning opportunities and the chance to empower individuals with disabilities to achieve their fullest potential. At MN Care Services, we are committed to always prioritizing person-centered care.

Hamza Muridi

Vice President

With 15 years of experience in the home healthcare field, Hamza has developed a deep expertise in delivering compassionate, patient-centered care. Throughout my career, I have worked across various facets of home healthcare, including skilled nursing, personal care services, and care coordination. Hamza has demonstrated a strong commitment to enhancing the quality of life for patients, ensuring their safety, comfort, and well-being in the home environment. Known for a patient-first approach, I have excel in building strong relationships with patients, families, and healthcare teams, and are adept at navigating complex healthcare needs with professionalism and empathy.


Natasha Flowers

Executive Assistant

With 16 years of experience in Disability Services, Natasha has built a comprehensive career that reflects her dedication and expertise. She began her journey as a Direct Support Professional (DSP), where she gained hands-on experience providing essential care and support. From there, Natasha advanced to the role of Behavioral Technician, further developing her skills in managing and supporting individuals with diverse needs. Ultimately, she achieved her goal of transitioning into an Administrative position, where she now applies her extensive knowledge to oversee operations and ensure the delivery of high-quality services. Natasha is deeply committed to helping those in need and continues to be a valuable asset to the field of Disability Services.


Martin Triviski

Compliance Director

A detail-oriented and compliance-driven professional with over a decade of progressive experience in the human services field. With a proven track record in quality assurance, risk management, and leadership, Martin excels at navigating regulatory environments and fostering operational integrity across all levels of service delivery.

Martin currently spearheads organizational initiatives to uphold and surpass compliance standards within a dynamic and increasingly complex regulatory environment. His previous roles were instrumental in shaping effective policies, designing robust training programs, and implementing internal audit systems that promote ongoing alignment with regulations.

He launched his career as a Direct Support Professional, gaining firsthand experience in supporting individuals with complex needs. This early, hands-on work continues to shape his compliance philosophy—grounded in both regulatory rigor and a deep commitment to compassionate, person-centered care.


Amy Sagstetter

SHRM-SCP, Human Resources

Amy brings with her 14 years of HR experience and over 20 years in administrative roles. Her career has spanned organizations of all sizes — from enterprise-level employers with 27,000 employees to small businesses with 140 — giving her a unique perspective and versatility in supporting teams.
She specializes in people-first HR operations, guiding employees from pre-employment through retirement. As Amy puts it:
“My favorite part of what I do is watching and guiding employees as they grow and thrive, whether they move into advanced field roles or take on leadership positions.”


Alyssa Thompson

Designated Manager

With an Associate of Applied Science (AAS) in Medical Assisting and over 13 years of diverse experience in residential care, I am dedicated to enhancing the quality of life for individuals through compassionate care and effective support systems. My career has spanned a variety of roles, from direct support to senior leadership, with a particular focus on crisis stabilization services for high-acuity youth and adults.

I am deeply passionate about promoting independence and empowering individuals to achieve their goals while living in their own homes. Known for my strong advocacy, I prioritize open communication and collaboration with teams to foster lasting relationships and ensure the best outcomes for those in our care.

Committed to continuous professional growth, I consistently seek opportunities to expand my skills and knowledge in order to better serve the individuals we support and make a meaningful impact within the residential care field.




Shannon Rivero

RN/BSN with clinical experience in multiple specialty areas. Strong compassion in caring for people and enjoy building relationships with people and their families while providing excellent patient care. Healthcare is a continuously changing field that I want to continue to be a part of for the good of all people who have to utilize it in any way. Being able to work both in the clinical bedside area as well as in the administrative side of nursing has enabled me to become a strong well-rounded nurse.

Does MN Care Services qualify people for services?

If you are an MA/Medicaid subscriber, then you will enjoy the support of a social or financial worker in your county. We do not offer financial work to qualify you but we can point you to the right office in your county to determine your eligibility. For private paying clients, yes, we would have sessions with you to agree your ability to pay and work out a mutually beneficial mode of payment.

Need Services? 

We provide care to those that have the following:

  • Waivers: Community Access for Disability Inclusion (CADI); Developmental Disabilities (DD); Brain Injury (BI); Community Alternative Care (CAC); Elderly Waiver (EW); Consumer Directed Community Supports (CDCS)

  • Private & Self Pay

  • County Funding

  • Most services are waiver eligible and payable through the Home and Community Based Waiter Program. Funding is accessed through Medical Assistance or PMAP programs.
